Background: Strabismus surgery aims to improve the ocular alignment of visual axis & cosmetic appearance studied cases with strabismus. The most common functional benefits are restoration binocular vision, elimination diplopia compensatory head posture.In large-angle exotropia, many surgical methods have been performed, bilateral lateral rectus recession, medial resection recession frequently used. Aim: To evaluate outcome supermaximal monocular recession-resection versus large in angle exotropia. Patient Methods: This was a prospective, interventional, nonrandomized study. study done at Ophthalmology department AL zahraa university hospital, Cairo, Egypt, duration from May, 2021 2022 and included (20) patients, they were divided into 2 groups: group A (10) who underwent recession- operation exotropia B patients Results: At final examination, In RR group, 7 10 (70%) successfully aligned 3 (30%) had recurrence. BLR 8 (80%) achieved successful alignment, (20%) Successful recurrence rates not significantly different between groups. Conclusion: rate 2-muscle patient 75% overall results comparable
